Registering with our practice is simple and secure. Here’s what to expect during the registration process—before any treatment is provided:

1. Complete New Patient Registration Forms

  • Fill out your REGISTRATION forms
  • Forms include your personal details, medical history, and dental history.

2. Provide Identification

  • If you’re registering under the NHS, please also bring your NHS number and proof of any exemption from NHS charges, if applicable.

3. NHS Exemption Information (If Applicable)

If you qualify for free NHS dental care (e.g. through pregnancy, income support, universal credit, etc.), you’ll need to:

  • Indicate this on your registration form.
  • Provide up-to-date proof of exemption (e.g. a valid exemption certificate, maternity exemption, or benefit documentation.

4. Booking Your First Appointment

Once registration is complete, we’ll help you schedule your initial appointment and confirm next steps
